Developed by Dr. Randolph Resnik and dentist-attorney Dr. Francis DeLuca, the “Medical Consultation for Dental Implant Surgery” form is designed to inform the patient’s physician about the intended dental implant surgical procedure, protects the dentist medicolegally, and provides the dentist with sufficient information about whether to proceed with treatment. This form, along with a cover letter template, is available for free download.
